Gem upgrade chances, for all we know, are fixed numbers and not "rounded", 100% means 100% and 0% means 0%. Those two bugs remain isolated incidents and might just have been bugs, like sometimes Death's Breath not dropping or so. I remember instances of people reporting 100% fails and it turned out to be a reading comprehension issue (they thought that a GR35 is supposed to give them a 100% upgrade chance on their rank 35 gem for some reason).
